PERMISSION SLIP FOR TRANSPORTATION
By agreeing to this document hereto:
1) I authorize and give permission for my child to be transported by Stubbs' Music Center, Inc. for music instruction, or to participate within a field trip.
2) I agree to release and hold harmless Stubbs' Music Center, Inc. from any liability to me or my child which may arise as a result of my child being transported.
3) I will notify Stubbs' Music Center, Inc. by ten a.m. if my child is not riding the van on the scheduled day.
4) If my child is not at designated waiting area within ten minutes after the school bell, Stubbs' Music Center, Inc. is not responsible for returning to school to pick-up my child.
5) If my child does not wear his seat belt, he or she will not be a permitted to continue to ride the van and no refund will be given.
6) On teacher planning days the Stubbs' Van does not pick up students due to scheduling.
