PERMISSION SLIP FOR TRANSPORTATION

By agreeing to this document hereto:
1)  I authorize and give permission for my child to be transported by Stubbs' Music Center, Inc. for music instruction, or to participate within a field trip.
2)  I agree to release and hold harmless Stubbs' Music Center, Inc. from any liability to me or my child which may arise as a result of my child being transported.
3)  I will notify Stubbs' Music Center, Inc. by ten a.m. if my child is not riding the van on the scheduled day.
4)  If my child is not at designated waiting area within ten minutes after the school bell, Stubbs' Music Center, Inc. is not responsible for returning to school to pick-up my child.
5)  If my child does not wear his seat belt, he or she will not be a permitted to continue to ride the van and no refund will be given. (Has been a major problem with middle school students.)
6) My child understands that he or she is not to leave any garbage or trash in the van.  Again, if the student does leave trash in the van, the parents will have a choice of one of the following options either the student will no longer ride the van with no refund being given or the parents will pay for the van to be cleaned $25.
7)  On teacher planning days and early release days I understand that Stubbs' Music Center will ONLY pickup if you call and request us to do so.
